Can diabetes be treated with surgery?

For selected patients with Type 2 diabetes and obesity, current society guidelines recognize metabolic surgery as an evidence-based treatment option. This page summarizes what that means — and what it does not.

What "treatment" means

Metabolic surgery can improve glycemic control through anatomic and neurohormonal changes that affect insulin sensitivity, appetite regulation, and nutrient handling. In published trials, surgery has produced greater and more durable A1c improvement than medical therapy alone in selected patients.

What it does not promise

Diabetes surgery is not a cure. Remission is possible for some patients but is not guaranteed, and relapse can occur. The MagDI™ System received FDA De Novo authorization for the creation of side-to-side duodeno-ileal anastomoses; FDA documentation states that the effects of the device on weight loss were not studied.

Updated society guidelines recognize metabolic surgery as an option for adults with BMI ≥ 30 and Type 2 diabetes who have not achieved durable glycemic and weight control with non-surgical therapy.
